The title page should contain the following information: (1) Title of the article; (2) names and initials of each author; (3) department and institution to which the work was conducted (4) name, address, telephone, fax and e-mail address of the author responsible for correspondence; (5) sources of financial support; (6) category in which the article is being submitted (case report, original articles, review etc); (7) keywords; (8) running title.
